Abstract: A system and method for facilitating diabetes management is provided. The method includes mobile/web application which is configured to receive the blood glucose readings from the patient either manually or by connecting the mobile to the blood glucose monitoring device. The readings are then transmitted to the healthcare-provider of the patient’s choice via the app. Further, the provider can then view the readings on the app along with other details such as patient profile, treatment information, allergy information etc. which is stored in a database. Furthermore, the system of the present invention includes mobile/web application running on a computing device operated by the patient and healthcare-provider; a server for storing patient profile and blood glucose readings. Moreover, the server also has the ability to track the location of the patient and suggest the suitable healthcare-provider.
