Body Of A Device For Positioning A Coronary Stent Or Coronary Balloon In Coronary Arteries

Abstract: A body of a device for positioning a coronary stent or coronary balloon in coronary arteries is intended for use in the field of medicine. The body consists of a front portion and a rear portion, each of which consists of two identical halves. The rear portion of the body overlappingly connects to the front portion of the body. At one end, the front portion is configured as a truncated portion with a tubular extension having a slot for connection to a Y connector; at the other end, the front portion transitions successively from a cylindrical portion into a smooth cylindrical portion of a smaller diameter and into a guide for a runner. In its front cylindrical portion, the body is provided on both sides with rectangular openings for tabs of a fixing mechanism, each opening having bump-like protuberances for holding the tabs of the fixing mechanism in an end position and for keeping the fixing mechanism itself in a closed state. A cylindrical groove with bump-like elements is provided around the circumference of the smooth cylindrical portion of the front portion of the body, at the runner guide end thereof. The inside surface of the rear portion of the body consists of a smooth surface and of a surface having a double start thread about the circumference thereof. The smooth surface of the rear portion of the body is disposed over the smooth cylindrical front portion of the body, and the inside surface having a double start thread about the circumference thereof for engagement with projecting portions of the runner is disposed over the runner. Between the smooth inside surface of the rear portion and the surface of the rear portion having an internal double start thread there is a cylindrical protuberance having bump-like elements for engagement with the cylindrical groove of the smooth cylindrical portion of the body such that the rear cylindrical portion is capable of rotating about its own axis. The outside surface of the rear portion of the body is provided with alternating horizontal smooth transparent strips and strips having a stippled surface. In the front portion of the body, in the tubular extension, and in the end wall of the rear portion of the body there are through openings to allow a coronary stent or balloon delivery system to be introduced into a device disposed inside the body. The end surface of the rear portion of the body has a bevelled edge.

The present invention is a housing of a device intended for use in medicine for the most accurate, fast and safe positioning of a coronary stent or coronary balloon on a delivery system during endovascular coronary stenting surgery.

Devices for positioning a stent in the coronary arteries of the same applicant (RU2598798, RU2652732, RU2650038, RU2661092) are known on the medical equipment market, the body of which differs from the claimed utility model having the following advantages:

- optimization of the positioning of the coronary stent or coronary balloon by increasing the length of the body and thus the range of movement of the fixed delivery system,

- increased efficiency by increasing the area of ​​engagement with the fingers of the rear of the body and the location of the hands as close to each other as possible due to the design in which the rear of the body is overlapped with the front,

- increasing the reliability and stability of the device due to the presence of point protrusions in the holes for the ears of the retainer in the front part of the housing, which allow keeping the retainer closed, thereby preventing the possibility of arbitrary release of the delivery system of the coronary stent or coronary balloon in the fixed state,

- increasing the reliability and efficiency of the operation due to the presence of horizontal stripes with a shagreen surface on the back of the body, which increases the adhesion of the body surface with the surgeon's fingers,

- the ability to visually control the operation of the mechanism inside the case due to the presence of horizontal transparent stripes alternating with shagreen stripes on the back of the case,

- reducing friction and improving the quality of rotation due to the presence of point elements on the surfaces of the body, rotating relative to each other,

- ensuring a firm and reliable fixation of the device when connected to the Y-connector due to the presence of the extension of the front part of the device in the form of a tube,

- ensuring a smooth and comfortable contact of the end of the back of the body with the surgeon's hand due to the presence of a chamfer,

- simplification of the design and reduction of manufacturing costs due to obtaining the halves of the rear part of the body identical as a result of the use of a two-start thread on the inner cylindrical surface of the rear part of the body.

The claimed invention is industrially applicable.

The subject of the present invention is a device housing for positioning a coronary stent or coronary balloon in coronary arteries.

The body contains a latch with a thickening with ears, a slider with an opening for placing the delivery system in it, with protrusions designed to engage with the internal two-start thread of the rear cylindrical part of the body, and protrusions designed to move along the guides inside the body on one side, and a cap with a hole for placing a delivery system with a rubber sleeve with a hole for placing a delivery system inside with a tapered and end surfaces corresponding to the same surfaces inside the cap and at the end of the slider, placed in the cap with a thread for screwing onto the slider on the other side.

The mechanism located in the housing is the subject of a separate patent for invention (patent RU 2661092) by the same applicant and is provided only for the purpose of illustration and understanding of the claimed utility model.

The body consists of the front part of the body and the rear part of the body, each of which consists of two identical halves connected to each other. The rear of the body is overlapped to the front of the body.

The front part of the body is, on the one hand, a truncated part with a continuation in the form of a tube, and on the other hand, it passes successively into a cylindrical part, a smooth cylindrical part of a smaller diameter, and a guide for the slider.

The front truncated part of the body is adapted to hold the fingers of the surgeon's left hand, and is provided with recesses that allow the surgeon to fix the device in the desired position, while the recesses also serve as a platform for fixing the coronary conductor. The recesses are made with a shagreen surface.

The front truncated part of the body is intended for connection to the Y-connector and is made with a continuation in the form of a tube with a cutout for the coronary conductor. An extension of the body in the form of a tube is inserted into the hole in the Y-connector, and the corona wire passes through the cutout in the tube and exits at the beginning of the notch on the outside of the front truncated part of the body.

This design allows you to accurately and firmly connect the device to the Y-connector and eliminate arbitrary displacement of the device body relative to the Y-connector during positioning of the coronary stent or coronary balloon.

This design also reduces the stress on the fingers of the surgeon's hand, facilitates and simplifies the operation.

As an option, the front truncated part of the body can be made without extension in the form of a tube and not intended for connection to the Y-connector.

The body in its front cylindrical part on both sides is provided with rectangular holes for the lugs of the retainer. Moreover, in each of the rectangular holes on one side at a distance equal to the width of the latch ear, there are two point projections designed to hold the lugs of the latch in the extreme position, and the latch itself in the closed state.

Around the circumference of the end face of the smooth cylindrical part of the front part of the housing from the side of the runner guide, a cylindrical groove with dotted elements is made, intended for connection with the corresponding protrusion with dotted elements, made around the circumference of the smooth inner cylindrical surface of the rear part of the housing from the side of the surface with a double thread.

The rear cylindrical part of the body from the inner side is made in the form of a smooth surface and a surface with a two-start thread around the circumference.

On the inner smooth cylindrical surface of the rear part of the housing from the side of the retainer, point elements are made around the circumference, designed to reduce the friction of the parts of the housing rotating relative to each other.

Between the smooth inner surface and the surface with an internal two-start thread of the rear part of the housing, a cylindrical protrusion with dotted elements is made around the circumference for engaging with a cylindrical groove of the smooth cylindrical part of the front part of the housing from the side of the runner guide with the possibility of rotation of the rear cylindrical part of the housing around its axis.

The outer surface of the rear part of the body is made with alternating horizontal smooth transparent stripes and stripes with a shagreen surface.

As an option, the outer surface of the rear part of the housing can be completely made with a shagreen surface.

When assembled, the smooth surface of the rear of the housing is positioned over the smooth cylindrical front of the housing. And the inner surface, made with a double thread around the circumference, designed to engage with the protrusions of the slider, is located above the slider.

The back of the body is overlapped over the front of the body, over a smooth cylindrical part with a detent inside, and over a slider.

A chamfer is made in the end surface of the rear part of the body, which provides a smooth and comfortable contact of the end surface with the surgeon's palm.

In the front part of the body, in continuation in the form of a tube provided with a cutout for the coronary conductor, and the end surface of the rear part of the body, through holes are made for introducing the delivery system of the coronary stent or coronary balloon inside the device placed in the body.

The body is made of plastic by injection molding and consists of a front and a back, each of which is made up of two identical halves.

Each half of the front truncated part, the cylindrical part and the smooth cylindrical part of the smaller diameter of the front part of the body and each half of the rear part of the body are cast entirely.

As an option, the halves of the rear of the housing can be made with an offset of the single-start thread of the inner surface to form a single threaded surface during assembly.

The halves are made separately and are connected to each other after assembly of the mechanism by means of tapered protrusions and corresponding tapered holes made along the edges of the front and rear parts of the inner part of the body.

When assembling the device, a rubber sleeve with a hole for placing the delivery system, a cap with an opening for placing the delivery system, and on the opposite side of the lock a slider with an opening for placing the delivery system is placed in the retainer on the side of the thickening, on the thread of which the cap is screwed, in which the rubber bushing. After that, a tube is inserted into the through hole of the sleeve, cap and slider to accommodate the delivery system of the coronary stent or coronary balloon.

The halves of the front part of the body are connected by means of conical protrusions and the corresponding conical holes made in the front part of the case, after which the smooth cylindrical part of the front part of the case is placed in the halves of the rear part of the case, and the cylindrical protrusion with dotted elements is located in the cylindrical groove of the smooth cylindrical part of the front parts of the body, and the halves are connected similarly to the front part of the body by means of tapered protrusions and corresponding tapered holes made in the rear part of the body, forming an overlapping connection with the front part of the body.

The body consists of the front part of the body 1 (Fig. 1, Fig. 2, Fig. 3, Fig. 4, Fig. 5, Fig. 7) and the rear part of the body 14 (Fig. 1, Fig. 2, Fig. 3, Fig. 4, Fig. 5, Fig. 6, Fig. 7), each of which consists of two identical halves. The rear part of the body 14 (Fig. 1, Fig. 2, Fig. 3, Fig. 4, Fig. 5, Fig. 6, Fig. 7, Fig. 8) is connected to the front part of the case 1 (Fig. 1, Fig. 2, Fig. 3, Fig. 4, Fig. 5) with an overlap.

The outer surface of the rear part of the body 14 (Fig. 1, Fig. 2, Fig. 3, Fig. 4, Fig. 5, Fig. 6, Fig. 7) is made with horizontal smooth transparent stripes 37 (Fig. 1, Fig. 2 , Fig. 3), alternating with stripes with a shagreen surface 15 (Fig. 1, Fig. 2, Fig. 3).

Alternatively, the outer surface of the rear of the housing 14 (Fig. 10)

can be completely made with the shagreen surface 43 (Fig. 10).

The rear part of the body 14 (Fig. 1, Fig. 2, Fig. 3, Fig. 4, Fig. 5, Fig. 6, Fig. 7) is overlapped over the front part of the body 1 (Fig. 4) over the smooth cylindrical part 9 (Fig. 5) with a retainer 28 (Fig. 4, Fig. 5, Fig. 7) inside, and above the slider 24 (Fig. 4, Fig. 5).

In the end surface 38 (Fig. 4, Fig. 10) of the rear part of the housing, a chamfer 16 is made (Fig. 2, Fig. 3, Fig. 6), which ensures smooth and comfortable contact of the end part of the housing with the surgeon's palm.

In the front part of the body in the continuation in the form of a tube 33 (Fig. 5, Fig. 7, Fig. 9), provided with a cutout 34 (Fig. 5, Fig. 7, Fig. 9) for the coronary conductor 31 (Fig. 7), and end surface 38 (Fig. 4, Fin 5) of the rear part of the housing 14 (Fig. 1, Fig. 2, Fig. 3, Fig. 4, Fig. 5, Fig. 6, Fig. 7) through holes 21 ( Fig. 4, Fig. 5, Fig. 6) for the introduction of the delivery system of the coronary stent or coronary balloon inside the device located in the housing.

The body is made of plastic by injection molding, consists of a front part 1 and a rear part 14 (Fig. 1, Fig. 2, Fig. 3, Fig. 5, Fig. 7), each of which is two identical halves.

Alternatively, the halves can be distinguished by the offset of the single-start thread 41 (Fig. 8) to form a single threaded surface during assembly.
