Abstract: A composition is disclosed for preparing a hydro gel and an ointment for treating patchy baldness on scalp and for causing new hair growth. The disclosed hydro gel and ointment, based on natural products allows new hair growth to be obtained within a very short time both in men and women. The present invention pertains to a composition containing croton seed kernel as a nonspecific irritant agent for treatment of alopecia areata in general and patchy baldness in particular. Croton seed kernel may be used in quantity depending on the area of the bald patch which is therapeutically effective, in combination with water, wax or petroleum jelly as vehicle. The composition of single dose combination of hydro gel and ointment and the procedure of treatment is disclosed. Preferably, before the composition of the present invention is applied to the scalp, the bald spots are scrubbed with cotton. This cleanses the residue from the skin. The composition of the present invention is then applied on the bald spots. The invention has found that it usually takes three-fifteen days for pores to open and fifteen days to three months for hair to grow from periphery to centre of the bald spot. The inventor has found that it takes approximately one to four months for hair to grow to the point where it appears normal in texture and colour. In all cases treated with the composition, successful results have been obtained.

TOPICAL COMPOSITION AND METHOD FOR HAIR RE-GROWTH ON
PATCHY BALD SPOTS
Technical field:
This invention concerns an herbal composition which is topically applicable and without exerting adverse reactions at the site. It contains a minimum, but therapeutically effective quantity of croton seed kernel in combination with a topically applicable vehicle. The composition is used for treating patchy baldness and promoting new hair growth within a very short time both in men and women and a method for use of composition to cure Alopecia Areata on scalp. The preparations of the composition are in suitable formulation as hydro gel and ointment for topical use in required quantity, directly on the bald spot on the scalp to be treated. Very successful results have been obtained.
Background of Invention:
Alopecia areata is a highly unpredictable, autoimmune skin condition that causes loss of scalp hair, facial hair, and hair elsewhere on the body It affects approximately 1.7% of individuals (over 4.7 million people) in the United ì¥Á 5@ ð ¿ RŠ
bjbjÏ2Ï2 jØ X X R‚ ÿÿ ÿÿ ÿÿ ˆ 6 6 6 6 6 6 6 ô * n n n n l he scalp. It can progress to total scalp hair loss (alopecia totalis) or total body hair loss (alopecia universalis). Alopecia areata affects males and females of all ages and races. Onset often begins in childhood when it can be emotionally devastating. Alopecia areata is not life-threatening, but is most certainly life-altering. Its sudden onset, recurrent episodes, and unpredictable course have profound psychological impact.
Alopecia may arise from many causes, including stress reactions, hypothyroidism, exposure of the hair follicles to topically-applied chemicals, chemotherapeutic drugs used for cancer, and genetic male-pattern balding. The disorder is often classified by its specific manifestation, such as patchy balding (alopecia areata), total loss of head hair (alopecia totalis), or total loss of body hair (alopecia universalis). Alopecia areata and alopecia totalis frequently affect women more than men.
Alopecia areata is the name for a condition in which round patches of hair loss appear suddenly. The hair-growing tissue is attacked by the patient's own immune cells for unknown reasons. It is non contagious and there is no relation to any food product. Because it causes bald spots on the scalp, it is sometimes called spot baldness.Area other
than scalp such as eyelashes, eyebrows, beard, pubic and body hair may be affected in 10% of patients by Alopecia Areata .It may also affect nails causing pitting and ridging in 10% of patients. Most of the patients suffer with Alopecia Areata belong to age 20 years and above but any age,sex or race may be affected. A family history may suggest it a hereditary disease in 10-20% of cases.
The cause of alopecia areata is unknown. The currently favored theory is the autoimmune theory According to this theory; the body's lymphocytes (a type of white cells) attack the hair follicles and cause hair to fall.Alopecia Areata is not a disease which will kill the patient like all systemic diseases which causes much physical disturbances and can be fatal if not treated, but it definitely causes worries and awkwardness for the patient to move in society. The bald patches on scalp draw the attention of passerby and also the people around him/her.
Till date many drugs belonging to different classes have been tried and still the trials are going on. There are Alopecia Areata Societies in USA and UK and other part of World where people suffering from Alopecia Areata come together and discuss the present and future treatment for their disorder and also enjoy life as every member is under stress and wants to find out for new solution.
The aims and objectives sub served by various treatment class of drugs treatment:
1. To reduce the stress, agony, embarrassment and depression in general; and
2. To re-grow the hair, in particular.
Drugs are used to treat Alopecia Areata with variable results. Still the Clinical Trials are going on all over the World. However, there is no permanent cure for alopecia areata and there is no universally proven therapy for inducing remission. There are a range of therapies for which partial success has been claimed but as soon as any of the therapies are stopped alopecia areata returns. All the current modalities used are more effective in those with milder forms of the disease but much less effective in people with extensive hair loss,
The various therapies tried so far and also used currently, can be categorized into several groups:
Non specific immunosuppressants: Corticosteroids, 8-methoxypsoralen.plus ultra violet A light (PUVA), phototherapy.
Intraregional steroids: Steroids are injected directly into the bald patches at monthly intervals until growth of hair becomes evident, this may take six to 12 weeks. The re-grown hairs may be light or white in colour but will darken as they grow longer. The injection is no more painful than injections elsewhere and there is no danger of the injection injuring the brain as injections are given subcutaneously, but intralesional injections of steroids may not be suitable for children because of the severe pain.
Strong steroids applied to the bald areas twice daily have been reported to produce regret in some patients. However, response is slow, taking sometimes up to six months. Still, topical steroids are convenient, painless and can be tried, especially in children who may not be ready to take injections. Steroids given orally are extremely effective, but the problem is of recurrence especially when treatment is discontinued. Therefore, treatment has to be continued six months or more. Prolonged steroid treatment with systemic steroids may cause side effects such as weight gain, easy skin bruising, upset stomach, and bone thinning and muscle weakness. Treatment should be under Close supervision of a doctor is necessary because of these side effects.
Ultravialet_B light: Ultraviolet_ B from an artificial light source has also been used to induce mild sunburn on the bald areas. I t is believed that the mild sunburn irritates and stimulates the hair follicles into producing hair again. However, this treatment is inconvenient because the patient has to go to the clinic regularly for treatment.
PUVA: Psoralen plus ultraviolet light-A, normally used for treating psoriasis, has also been used in patients with alopecia areata. This treatment is repeated two to five times a week at the clinic for a total of about 20 treatments; the results have been variable with some doctors reporting benefit and others reporting none. Like ultraviolet-Blight, this treatment requires the patient to go to the clinic regularly for treatment; Special precautions have to be taken to avoid direct sunlight for several hours after treatment. PUV A has a depleting effect on Langerhan cells (another type of cell that is involved in immune reactions) and this somehow encourages hair growth.
Isoprinosine: This is an immunomodulatory drug (a drug which has effects on the immune system). It has been reported to induce re-growth in patients with alopecia areata who have laboratory evidence of altered immunity. Isoprinosine may cause an increase in uric acid, causing gout; so monitoring of uric acid levels is a necessary precaution during treatment. Asprin poultice has an immunomodulatory effect.
Treatments of direct action on the hair follicle- Minoxidil: Minoxidil lotion was originally developed for the treatment of male pattern baldness. A few small-scale studies
suggest that minoxidil can induce re-growth in 50-80°/0 of patients with alopecia areata. Minoxidil is believed to work by directly stimulating the hair follicles or modifying the immune reaction responsible for the hair loss. Minoxidil does not work very well in patients with severe alopecia areata, alopecia totalis and alopecia universalis, which are genuinely difficult to treat.
Non specific irritants: Anthralin, dithranol, croton oil, Mustard seed/Capsicum poultice, crude onion juice. These are applied to the bald areas to irritate and stimulate hibernating hair follicles to produce hair again. Dithranol cream for treating a skin condition known as psoriasis has been used with some success and regrowth has been reported in a few patients after about two months. The disadvantage 0f dithranol is that the cream is somewhat irritating to the skin and causes itching and, occasionally, burning. It may also stain the clothes and skin. A variety of other irritants such as phenol, benzyl benzoate, sodium lauryl sulphate and have also been used. Irritants have to be applied regularly to maintain the irritation. Treatment is uncomfortable and hence not very popular with patients.
Contact dermatitis inducers: Dinitrochlorobenzene (DNCB), diphenylcyclopropenon (DPCP), squaric acid dibutyl ester (SADBE).Topical allergens: Instead of irritation, this method uses allergens to cause an allergic reaction and to stimulate the follicles into producing hair. A number of allergens have been used, for example, DNCB (dinitrochlorbenzene), SADBE (squaric acid dibutylester), diphencyprone, poison ivy resin and primula leaves.application of anyone of these will induce allergy in most people. If the same substance (allergen) is applied to the skin again, an allergic reaction will develop at the site of application. Repeated applications maintain the reaction, and about 25_50% of patients with severe alopecia areata have shown re-growth three to six months later.lt is believed that the allergic reaction induces the production of a group of lymphocytes known as suppressor_ T cells which suppress the immune reaction responsible for the hair loss. Topical allergens definitely work because some patients who have been totally bald for years have re-grown hair. the problem is the reaction can be very uncomfortable and at times associated with painful swelling of the lymph glands in the neck and fever. DNCB is less popular now because laboratory tests show that it may be cancer-causing.
Specific immunosuppressants: CycJosporine (CyA), Neoral, Tacrolimus (FK506), sulfasalazine. Cyclosporin: Cyclosporin is a drug used to prevent the rejection of transplanted organs. It suppresses the immune systen and also causes hah- growth as a side effect.Oral cyclosporin has been reported to induce re-growth in some patients with alopecia areata. It may work by suppressing the immune reaction that is causing the hair
loss or by directly stimulating hair follicle growth. Because cyclosporine is toxic when taken by mouth, doctors have tried using topical cyclosporin instead. Unfortunately, only mild re-growth was observed. Like in male pattern baldness, this may be related to the inability of cyclosporine to penetrate the skin and reach the follicles.Doctors are experimenting with other formulations in an attempt to improve the penetration of cyclosporine into the skin.
Various alternative treatments were tried for example: Stress reduction.Aloe vera, Poison ivy, Melatonin, Sunlight, Heat treatment, Zinc supplements, Vitamin supplements/creams, Dimethylsulfoxide (DMSO) - a hydroxyl radical scavenger, and antioxidant, essential oils, Chinese Herbs. Evening primrose oil (EPO), borage and black currant oil, Flax seed oil, linseed oil, fish oil. Homeopathic treatment - "Phosphoricum acidum", is prescribed for the treatment of baldness and showed hair growth but recurrence of hair loss when treatment stops. These therapies are used in different parts of the world with varied results.
Experimental treatments: cytokines, biologicals.
Theoretical treatments: Desensitization, gene therapy, new immunosuppressant drugs. Aromatherapy and acupuncture
Hair Transplantation: Transplant productive hair follicles from a donor area on the scalp to a balding area. Hair follicles are taken from the sides or back of the head and moved to the front and/or top, slowly reconstructing a hairline. Some scarring at the donor site may result. Transplanted hair follicles can be permanent or may be lasting for few years depending upon the acceptability.
Scalp Reduction-Balding scalp areas can be surgically removed to decrease an appearance of baldness.
The above listed treatments are not warranted because of the serious side effects of drugs e.g. corticosteroids as well as from a cost stand point unless there is serious emotional or economic impact on the patient as a result of the disease. Many Inventors have suggested various mechanisms as per the above class of drugs used by them; many formulations were tried for the treatment of Alopecia Areata, but with very little success or with recurrence after the therapy were stopped.
Accordingly, there is always a need for a simple, inexpensive, reliable treatment for alopecia areata.
Disclosure of invention:
Against the described background, it is therefore a general object of the invention to improve the hair growth with a medicinal preparations act in curing patchy baldness (Alopecia Areata).
Very few reports are available that Croton oil was also used as non specific irritants, but because of high dose and direct use of croton oil, there were variable results which prompted the inventors to make the opinion that it should not be used as therapy for Alopecia Areata.
But contradictory to this opinion it was surprisingly found that Croton seed is used to treat Alopecia Areata (in local language "Undri or cliai") by villagers as folk medicine.
The source of the composition used by inventor is Croton tiglium, a small tree or shrub belongs to Family Euphobiaceae .The fruit of croton tiglium has smooth capsule of the size of a filbert, three cells, each containing a single seed these seed resemble castor beans in size and structure, oblong, rounded at the extremities with two faces; the kernel or endosperm is yellowish brown and abounds in oil. The oil is obtained by expression from the seeds previously deprived of the shell.
Seed of Croton Tig-Hum Linne (nat. ord. Euphorbiaceae), Croton tiglium, "Gayapala," Croton Oil. - A fixed oil expressed from seed.
They are 13 mm. long, 8 mm. broad, ovoid and bluntly oblong, covered with a brown shell, which on scraping becomes black. The kernel is white and oily. They yield 50 to 6o per cent of Croton Oil, It is a pale yellow or brownish-yellow, somewhat viscid, and slightly fluorescent liquid of Sp. gr., 0.940 to 0.960, having a slight fatty odor, and a mild, oily, afterwards acrid and burning taste.
Croton oil consists of:
1. The glycerides of stearic, palmitic, myristic, lauric and oleic acids; there are also present in the form of glycerin ethers the more volatile acids as formic, acetic, isobutyric and isovalerianic acids(l per cent in all; these give the odor. Tiglinic Acid (C5H802) is the characteristic one. The active principle is believed to be Crotonic acid, which is freely soluble in alcohol;
2. Several fatty acids, both free and combined to form fats; and
3. Crotonol (C18H2804), a substance which is non-purgative, but is capable of causing cutaneous irritation when applied to the skin, it produces rubefaction or a pustular eruption.
It is soluble ,when fresh, in about 60 parts of Alcohol, the solubility increasing by age; feebly soluble in Ether, Chloroform, Carbon Disulphide, and in fixed or volatile oils. The oil should be at least two years old; when fresh it is of no value.
The name Croton comes from the seeds, which resemble the "dog tick" or kroton in Greek. Tiglium, in turn, is thought to be derived from the Greek word tilos, meaning diarrhea.
Croton oil is a powerful drastic purgative, in large doses causes vomiting and severe griping pains capable of producing fatal effects. It acts with great rapidity, frequently evacuating the bowels in less than an hour. It is used to treat very obstinate constipation in lunatics who refuse to take anything , for very severe constipation due to lead poisoning, fecal impaction and for unconscious patients, from apoplexy Only one dose of one or two drops should be placed on tongue and being successful where other drugs have failed. It should always be used with the greatest care and should never be given to children or pregnant women, to those suffering from haemorrhoids, peritonitis, gastritis, or enteritis.
Croton oil was formerly used externally as an irritant and a counter-irritant for inflamed joints, in rheumatism gout, neuralgia, pleurisy, bronchitis, phthisis, etc.; but it is not often so used now, as having more disadvantages and availability of better drugs. Croton oil was used as Corson's paint is a 5 to 15 per cent solution of croton oil in ether, to destroy an inveterate patch of ringworm when all other remedies have failed.
Recently it is used in cosmetic products in very low concentration with carbolic acid to beautify skin that had been ravaged by age in combination.
Though this plant is categorized under toxic plants but the toxic effect depends upon the route of administration and quantity of the seed is used. This object is achieved by providing a combination of a medicinal preparation:
1. Part of kernel from croton seed mixed with water (Hydro gel), and
2. With soft paraffin wax /petroleum jelly (ointment).
When a composition is prepared with effective proportion, it acts at a faster rate, without producing any adverse reaction like pain and no systemic action like diarrhea, as used externally and has 100% success in hair growth.
Description of invention:
Croton oil was used earlier by some researchers but with very less success or no effect. Further they were not advocating the use of croton oil because of its toxicity. With recent reports of use of croton oil in cosmetic products prompted the inventor to use croton seed in treatment of Alopecia Areata.
The Inventor has used one fourth piece of the kernel of one seed to prepare the composition and used it without any toxic effects. Further the composition which was prepared for use was having part of kernel as a whole and not 'oil' which is used in . other inventions. As expected from the much diluted solution of fraction of kernel used in this invention, it gave the desired effect as growth of hair on baid spots.
The present invention describes a new approach in the treatment of Alopecia Areata of scalp over the prior art by using croton kermel as a whole by making a pulp and not extracted oil for hair growth on bald spot as described in prior art. The exact amount of composition to be used and duration of treatment depends upon the extent and nature of alopecia areata and the patient's response to it. The mechanism of action is unknown but basically it acts by inhibiting normal cell growth and its differentiation in the skin, modulating hair follicle inflammation and promoting hair growth.
The present invention is a method to one time use to promote the growth of hair on an alopecia spot on scalp with 98-100% success, without any side effects which are related to other therapies and there were no systemic adverse action like diarrhea, as it is used externally.
The present invention also focuses on a unique single dose composition comprising of the active ingredient incorporated with the vehicle and dispensed as hydro gel or ointment for topical application and external use only. The composition has following advantages:
• Croton seed kernel ointment or a hydro gel can be prepared by simple and an ordinary method, which does not, required expertise training.
• The composition can be dispensed in small collapsible tubes.
• The contents of the collapsible tube can be applied directly on the bald spot on scalp. Only required quantity of the composition can be used as per the size of bald spot and spread uniformly with a cotton bud.
• Croton seed kernel hydro gel /ointment with manual application without dressing,
• There is no complicated surgery or anesthesia involved and therefore needs no hospitalization. Discomfort and pain are minimal.
• As the composition is to be used as only one time application, the compliance will be better. This is very important from the point of view of convenience.
• Repeat application can be done on these areas if required after follow up for 8-10 days, if there is no hair growth.
• The composition of this invention is more advantageous from other therapies as it does not require any skilled assistance for application.
• The composition has no systemic toxic side effect as very small quantity is used to make the composition and as it is for external application only.
• The composition of the invention is prepared in required minute quantity so toxicity of active ingredient is not observed.
• The period required to show the effect as hair growth varies from 8 to 10 days, which is very short as compared to other therapies.
• Mild analgesics (pain killer) may be required if person feels pain at the bald spot due to inflammation produced by composition application. No other drug is needed which will reduced the cost of the therapy.
• Other bald spots can be covered in multiple sittings. It will be very useful especially in children who fear the intralesional injections of steroids.
• It can be considered as one of the alternate method to promote the hair re-growth especially on bald patch on scalp where hair are present encircling the patch.
• The hair restorer can be used to treat alopecia areata with total effective rate of higher than 99%, on permanent basis without recurrence.
This invention can be dispensed with warnings like-
1. To be used within short period after the date of manufacturing
2. To be applied on the bald spot with an applicator only
3. Hands should be washed with soap and water after the application to avoid food / water contamination
4. Keep out of reach of children.
Although various treatments, such as topical compositions, have been used to treat and promote the growth of new hair in patients with alopecia Areata, these prior compositions suffer from various problems which include lack of effectiveness, slow progress, more
time consuming and noncompliance on part of children. Therefore, there remains a need
for an effective and safe composition which will cure Alopecia Areata permanently,
The following examples are included to demonstrate preferred embodiments of the present invention. For those of skill in the art, that the techniques disclosed in the examples which follow, represent techniques to function well in the practice of invention and thus can be considered to constitute preferred mode for its practice. However those of skill in the art should, in the light of present disclosure, appreciate that many changes can be made in the specific embodiments which are disclosed and still obtain a like or similar result without departing from the spirit and scope of invention.
Example 1 (Case no.l):
Treatment of Alopecia Areata on scalp with Croton hydro gel(croton seed kernel with water) A young college student, unmarried girl, of 25 years age, getting ready to go for college, suddenly noticed a very smooth hairless spot on her scalp while combing her hair. The hairless spot of 2cm diameter was diagnosed as alopecia areata .After consultation with skin specialist and considering all pros and cones of other various available treatments, this composition of the present invention was used in the same manner as described above .A composition of croton seed kernel with water, paste was made by grinding a very small piece of seed in sufficient water. The fresh paste was applied to the bald spot on scalp with cotton bud, just sufficient to cover the spot. She was instructed not to have Hair wash for one week.
Example 2 (Case no.2):
Treatment of Alopecia Areata on scalp with Croton ointment (croton seed kernel with wax ) A young lady, aged 30 years, married with two children, suddenly noticed two bald spots on scalp while combing her hair. Both the spots were adjacent with 1.5 cm diameter and 1cm diameter respectively. The treatment was performed in the same manner as described above, but the composition was an ointment of croton seed kernel with wax which was made by grinding seed kernel in wax. The ointment was applied to the bald spot on scalp with cotton bud, just sufficient to cover the spot. She was instructed not to have Hair wash for one week.
Example 3 (Case no. 3):
Treatment of Alopecia Areata on scalp, with Croton ointment (croton seed kernel with petroleum jelly) A young boy of 12 years age noticed bald spot on scalp while oiling his
hair. It was diagnosed as alopecia areata. The spot was of 2cm diameter. The treatment was performed in the same manner as described above, but the composition was an ointment of croton seed kernel with petroleum jelly which was made by grinding seed kernel in petroleum jelly. The ointment was applied to the bald spot on scalp with cotton bud, just sufficient to cover the spot. He was instructed not to have Hair wash for one week.
Observation:
Daily follow up record was kept for All 3 cases to observe any adverse reaction, which were found to be nil. The Inventor observed in all 3 Cases: Swelling and erhythema developed on the bald spot, after 24 hours. On 3-4 day, it was further developed in vesicle and pustules on the spot, the oozing was gently dapped with cotton and coconut oil was applied on the spot to reduce the scratching urge because of itching sensation on the spot due to stretching and irritation of nerves at the site, which reduced after 4-5 days .Subsequently pores became visible. By 9 to 10 days hair gradually started growing in size and became thicker in diameter and darker in colour. The newly grown hairs were of same texture and shed as that of surrounding hair.
Result:
All 3 cases developed excellent (95-98%) re-growth of hair within 1 month.
Summary of invention:
A composition of the present invention is applied to the skin topically by any means of topical application known in the art. A composition of the present invention can be applied once on one bald spot at a time, if the number of bald spots is more than one, to avoid pain due to inflammatory action of invention. A composition of the present invention is preferably applied on spot that has been gently cleaned with cotton prior application of composition.
A composition, for external application - to stimulate hair growth - as one kind of traditional Indian medical preparation. The hair growth stimulator is prepared as:
(1) a hydro gel from water, and seed (kernel) of Croton Tiglium, and
(2) an ointment from wax/ petroleum jelly and Seed (Kernel) of Croton Tiglium by crushing a small portion of seed kernel and mixing it with water, soft paraffin wax or petroleum jelly.
The composition of the present invention is applied only on the bald spots; any excess composition is cleaned with cotton.
A composition of the present invention is applied in appropriate volumes such as can be readily determined by one of ordinary skill in the art. For example, a preferred volume of a composition of the present invention to be applied to an alopecia areata spot is just sufficient to cover the bald spot. This is very important to use composition within few days of its preparation date as older preparations may increase the adverse effects.
From next day coconut oil is applied to spot to reduce itch and so scratching on the spot. Hair wash is not allowed for eight days. The spots are observed daily to see the effect .On second -third day the erythema appears and on fourth- fifth day pustules appear. The inventor had observed that it usually takes five-six days for hair follicles to open and ten-fifteen days for hair to appear.
The inventor has found that it takes approximately one month for hair to grow to the point where it appears normal. The exact concentration and duration of treatment depends upon the extent and nature of alopecia areata and the patient's response to it. The newly grown hairs were of same texture and shed as that of surrounding hair, and leave no healing scar on scalp.
The present invention provides novel composition formulations which comprise croton seed kernel and vehicle. The invention also provides a method for preparing the hydro gel, and ointment and the use of the composition in products for topical application to the scalp. The products are very mild to the skin and may be easily formulated into a wide variety of personal care and dermatological products.
