2) The system as claimed in claim 1, wherein the said communication unit communicates through internet, smart card, fax, or in person with the display medium being a PC monitor screen.
3) The system as claimed in claim 1, wherein the said validation unit validates all the data of the patients such as drug data, laboratory results, physicians' assessments, and other data elements.
4) The system as claimed in claim 1, wherein the said all the health care data is maintained in the form of electronic health record (EHR) and stored in the internal database of the website.
5) The system as claimed in claim 1, wherein the said electronic health record includes all the data of the patient including clinical information about an individual patient and may include data from various primary stakeholders in the healthcare system: Payers (insurance companies and other entities at financial risk for care), providers (physicians, pharmacists, nurses and other medical professionals in nursing home), and the patients themselves.
, Description:FIELD OF THE INVENTION:
The present invention relates to electronic health records data management systems. The present invention more particularly relates to a system for maintaining healthcare management.
BACKGROUND OF THE INVENTION:
The Electronic Health Record (EHR) is for managing health care delivery. It is the most important tool for sick persons which require multiple specialists in multiple conditions. Powerful medications are needed for these people and careful care coordination is also needed from case and disease managers. These individuals are personally least able to communicate the complexity of their histories and health status to their next treating physician.
So the efforts are needed for reducing errors of omission and commission and to minimize the cost of duplicative and otherwise unnecessary care. Three persons give total information about their health are the patients themselves/ their caretakers, the patient’s physicians, hospitals and other providers like health policies insurance persons or any other payer.
Most patients have only limited information about their own care and even less ability to obtain, retain and store such data. Worse, a patient's ability to personally maintain their own health record decreases with illness, infirmity and, often, with age. Even the most user-friendly personal health record (PHR) systems available today are less frequently updated on a timely basis by their owners.
Physicians, hospitals and other providers are required by law and professional to maintain significant records pertaining to the care they provide. These providers do not obtain patient data from the spectrum of other providers. Thus, hospitals have a deep reservoir of information regarding the services and tests provided to patients within the facility and by the admitting physician, but some information is collected from other facilities or physicians who have treated those same patients. A single physician has records of everything about the patient as per their discussion and the treatment the doctor has provided, but the consulted doctor do not know what the patient has been told by other physicians nor what treatment other physicians have provided. So all the information remains paper based and hand written and difficult to integrate, analyze and/or transmit effectively.
Therefore, there is a need for a system for integrating patient data and providing it in the form of database who need it as per patient’s requirement.
OBJECTS OF THE INVENTION:
An object of the present invention is to provide a system for integrating patient data and providing it in the form of database who need it as per patient’s requirement.
Another object of the present invention is to provide an effective system for analyzing and integrating all the healthcare data.
Other objects and benefits of the present invention will be more apparent from the following description, which is not intended to bind the scope of the present invention.

The present invention is a system for maintaining healthcare management. The present invention provides a system for integrating patient data and providing it in the form of database who need it as per patient’s requirement and to provide an effective system for analyzing and integrating all the healthcare data.
In one embodiment, the electronic health record (EHR) is created and stored in the internal database of the website. All the data of the patients is collected and record is taken for carrying out present treatment opportunities, strategies and plans for the physician and patient. All the information of the patient is taken and record is maintained in the database of the website.
In another embodiment, electronic health record includes all the data of the patient including clinical information about an individual patient and may include data from various primary stakeholders in the healthcare system: Payers (insurance companies and other entities at financial risk for care), providers (physicians, pharmacists, nurses and other medical professionals in nursing home), and the patients themselves. The insurance company health record includes claims, self-surveys and other data. All the hospitals data is electronic medical record of all the patients which include doctor prescriptions, laboratory results, radiology scanning (if any) and other data of the patient.
Patient’s personal file is called as personal health record which include family medical history of the diseases (if any), patient‘s medical history and allergies (if any) patient is suffering from and any chronic disease and all health information reports related to various body parts.

Validation unit examines the aggregate record for duplicate and overlapping data (e.g. identical laboratory results from both the lab and the doctor's office), identifies that data, eliminates the duplicate data, and assembles a revised single, consolidated record of the patient. Internal validation processes are performed using various techniques, which may include probability assessment, referential edits, and other techniques to determine that certain key data elements in the consolidated record are, correct. For example, for some medical conditions such as heart attack, the “rule out” and “present” codes may be the same. An internal validation process is applied to drug data, laboratory results, physicians' assessments, and other data elements to conclude that a condition is present or not. Consolidated clinical history is provided for the correct patient. This record can then be used as a common, central electronic health record or EHR.
Inspection compares the consolidated patient record to evidence-based guidelines and identify care being delivered that is not appropriate as well as recommended care. Alerts, warnings, and potential errors that supports the physician's decision-making process are taken care of. Costing of the services required for the patients is assumed and told to the patients.
In an embodiment, the summary is also forwarded to authorized individuals. Communication can be via the internet, smart card, fax, or in person with the display medium being a PC monitor screen, or paper.
