Documents required for FSSAI

Any business involved in food operations must get itself registered with FSSAI as per the nature of its business. FSSAI registration/ license can help ensure the legality of the food operator, ensuring customers that the food is safe and hygienic. 

To obtain registration/ license, the food operator needs to apply in the following forms:

Annual turnover

Form

Below 12 Lakhs

FSSAI Registration Form A

12 Lakhs up to 20 Lakhs

State FSSAI License Form B

Above 20 Lakhs

Central FSSAI License Form B

Details of Documentation

This write-up describes the details of documents mandatory for Basic Registration and State Registration under FSSAI.

Documents mandatory for Basic FSSAI Registration:

  • Form A/B has been completed and appropriately signed. Where Form A is for Basic Registration & Form B is for State/Central Registration
  • Photo Identity (of Owner/Proprietor/Partner/Director of FBO)
  • 2 Photographs (Passport Size)
  • Documents for proof of possession of premises (e.g., Rental Agreement/ Utility bills, etc.)
  • Deed of Partnership/COI/MOA etc.
  • Particulars of food products to be dealt with
  • Food Safety Management System Plan

For State License:

  • Details (with name, ID, complete address, and contact) for:
  • Directors or 
  • Partners or
  • Proprietor or
  • Members of Society or 
  • Trust
  • Authorised Signatory
  • List of:
  • Machinery/equipment (their registration number, working capacity, and power usage)
  • Food to be manufactured
  • Certificate by Cooperatives (Coop – 1861/Multi-State Coop Act – 2002)
  • Declaration/Undertaking by Food operators
  • Form B (attested by the Proprietor/Partner/Authorized member)
  • Authority letter by the manufacturer nominating the person (along with details of the alternative person)
  • Self-Declaration (for Proprietorships)
  • Copy of License from manufacturer
  • NOC from manufacturer
  • NOC from Municipality or local body
  • Food Safety Management System Plan
  • Form IX: Details of persons nominated by the Company and copy of Board Resolution
  • Analysis report of water portability to be used in food. (for Manufacturers)
  • Where milk and milk products are processed, we need to provide a source/ acquisition plan of milk detailing the location of milk collection centers. (for Manufacturers)
  • Details of raw material sources for meat processing plants. (for Manufacturers)
  • Pesticide residue report of water. (for Manufacturers)
  • Proof of premises possession. (for Manufacturers)
  • Partnership Deed (for Partnership)
  • Affidavit of ownership/MOA/AOA of the firm (for Partnership)
  • Ministry of Tourism Certificate (for Transportation)
  • Self-declaration of vehicles (for Transportation)

Food operators must submit accurate information in the online application form along with the signed/attested documents (as required). The concerned department of FSSAI will undertake food quality inspection and premise inspection before issuing the License/ Registration Certificate.
