Abstract
ABSTRACT:Title: Artificial Intelligence Based System for Health Assessment and Care Continuum and Method ThereofThe present disclosure proposesan artificial intelligence based system for health assessment and care continuum and method thereof that provides a comprehensive approach for assessment of health and provides complete care continuum to individuals. The system 100 comprises of an automated query module 102, a health assessment module 104, a result collection module 106, a risk prediction module 108, and a reminder module 110. The proposed system utilises an electronic health record (EMR) platform that captures medical history and diagnostic results, physician impressions. The proposed comprehensive health assessment method provides artificial intelligence-based risk scores for cardiac disease, diabetes, pulmonary disease and other diseases. The proposed system ensures user stays on track as per goals set by physician personalized nudge-based care continuum using graded multimodal follow ups.
